  • 3 pcs. 1×1 Size Picture (white background with collar)
  • Medical Certificate (Fit for Training)

  • Certificate of Employment (for workers)
  • Driver’s License (for Driving NC II) photocopy
  • Application Form (TESDA-OP-CO-05 Competency Assessment Form)
